CC Authorization for Variable Charges


You authorize regularly scheduled charges to your Credit Card. A receipt for each payment will be provided to you and the charge will appear on your Credit Card Statement. You will receive an invoice 5 days prior to your credit card being charged. You agree that if you do not dispute the invoice within 5 days that the invoice/charges are valid. You agree to a 3.5% surcharge fee on each transaction.

I authorize AltrueTECH LLC to charge my Credit Card according to the terms.
